Dues Participation in the Provisional Program costs $280. This is the total of the Contract and Registration Fee ($110.00) and Provisional Dues ($170.00) for the year. The Contract and Registration Fee of $110.00 is due upon submission of my application (either on-line, in person, or by mail). Applications will not be processed without the payment of the Contract and Registration Fee. The Contract and Registration Fee is non-refundable. Provisional Dues of $170.00 will be due on October 25, 2017. By completing this application I agree to the terms of the above mentioned payment schedule. I understand that I may always pre-pay my Provisional Dues and that I can always contact accounting@jlsl.org to set up a payment plan. Provisional Program Requirements for Junior League of St. Louis I understand and commit myself to the completion of the Provisional Program requirements, including but not limited to: - Attendance at two (2) General Membership Meetings
- Attendance at four (4) Provisional Meetings
- Participation and completion of one (1) Cinnamon Roll Shift
- Participation and completion of one (1) Fundraising Shift
- Participation and completion of two (2) Community Impact Shifts
- Completion of one (1) Training Credit
- Serving on a JLSL Team and completing four (4) hours through meeting attendance and project work on that Team
- Completion of the Placement Process in Spring 2018
- Payment of JLSL Dues
I understand that I may always check my Program Requirement status on my member page of www.jlsl.org under Obligations. The estimated time commitment for this Program is less than 40 hours with many opportunities to fulfill requirements throughout the year. I understand that I may begin working towards fulfilling my requirements as soon as I submit my application and registration fee, but that all requirements must be completed prior to May 31, 2018. Active Membership I understand that, at the completion of the League year, the Provisional/1st Year Active Advisory Committee and Nominating Committee will review my completion of the Provisional Program Requirements and, if satisfactorily completed, will recommend to the Junior League of St. Louis Board of Governance my ratification as an Active member. I understand that if I do NOT complete all of my Provisional Program Requirements, I will not be eligible for Active membership status nor will I be reimbursed any fees or dues. I understand that having completed the Provisional Program Requirements and having been granted Active membership status, I must complete all Active requirements during the following year to remain an Active member. Finally, I understand that my Active dues will be billed on March 31, 2018 and must be paid prior to April 25, 2018 or I must have a payment plan in place by that time to maintain my Active membership status. General I understand that payment of my Junior League of St. Louis account is due by the 25th of the following month. I agree to pay the account when due. I give the Junior League of St. Louis and the Association of Junior Leagues International Inc. permission to publish in print, electronic, or video format the likeness or image of myself. I release all claims against the Junior League of St. Louis and the Association of Junior Leagues International Inc. with respect to copyright ownership and publication including any claim for compensation related to the use of the materials. |
